Lifestyle Modification Program - Prevention of Type 2 Diabetes

The Federal Government has announced a Type 2 Diabetes Prevention Program.  This program is aimed at people aged 40-49 years who are at risk of developing Type 2 Diabetes.

Download the "Australian Type 2 Diabetes Risk Assessment Tool".  (Go to Publications and Forms above).

Complete the tool - if you score 12 or more points it is important that you make an appointment to discuss your score with your GP - you may be eligible to be referred to the Lifestyle Modification Program. 

The Lifestyle Modification Program is a group program which aims to teach participants about healthy eating, physical activity, stress reduction and goal setting as a way of supporting lifestyle changes and adopting healthy lifestyle behaviours.
